Can Steroids and Protein Powders Damage Hip Bones Permanently?

Orthopaedic experts are seeing hip bone damage and degeneration—typically associated with the elderly—emerging among young people in their 20s. This trend is being linked to the arbitrary use of performance-enhancing steroids and protein powders commonly obtained from gyms.

Recently, the Maharashtra FDA launched statewide checks on protein powders after several brands were found to be spiked with performance-enhancing steroids. Many of these substances are freely available online or sold at gyms without proper labeling or approvals.

According to Dr. Gaurav Prakash Bhardwaj, Director, Sports Injury, Joint Preservation and Replacement Surgery, PSRI Hospital: “Steroid use is one of the most well-documented causes of Avascular Necrosis (AVN), a condition where bone tissue dies because of lack of blood supply. When taken in high doses or over a long period—especially without medical need—steroids can interfere with the blood supply to the bone.”

What kind of hip-related complaints are you seeing among young patients?

There has been a noticeable rise in hip-related complaints among young individuals, especially those in their 20s and early 30s. These patients report persistent pain in the hip joint, difficulty walking, or stiffness—symptoms often linked to unsupervised gym workouts and supplement use. Abuse of steroids can also negatively impact liver and cardiovascular health.

How often are young patients diagnosed with AVN?

Previously, AVN was considered relatively rare in this age group. However, in recent years, urban hospitals are reporting 3 to 5 new cases per month. Many young individuals are unaware that their symptoms indicate a serious condition until irreversible damage has already occurred. While medication may slow down the degeneration, surgery often becomes the last resort.

How does alcohol and steroid use contribute to AVN among young adults?

Steroid use is one of the leading causes of AVN. When used excessively or without a medical reason, steroids can elevate fat levels in the blood and bone marrow, which obstruct blood vessels. This impairs blood flow to the bone, eventually leading to bone tissue death and collapse of the hip joint. Similarly, excessive alcohol intake also damages blood vessels and reduces blood flow to the hip, contributing to the development of AVN.

Why do young people depend on performance-enhancing drugs?

The most affected age group falls between 20 and 35 years. These are typically fitness-conscious individuals driven by intense workouts and influenced by social media trends or body-building goals. Unfortunately, many are using steroids or unregulated protein supplements without medical supervision. There is an urgent need to regulate the supplement market, maintain hygiene standards in gyms, and implement basic bone screening for gym-goers.
